Premier Point Home Health Services

Premier Point's on-staff physical and occupational therapists will coordinate with physicians. Our therapists will discuss with the patient the severity of the pain, injury, or disability, assess current functionality, and develop a therapy regimen aimed at improving, adapting or restoring functions which have been impaired or permanently lost with the goal of achieving measurable improvements. As part of the care plan development process, our therapists work with patients to establish realistic goals, and determine how many therapy visits it may take to reach desired outcomes.

Physical Therapy

Premier Point’s physical therapists develop customized care plans based upon the specific condition presented. Plans offer a broad array of therapy techniques including directed and monitored endurance exercises; specific repeated movements to reduce symptoms; strengthening and stabilization exercises; and appropriate education regarding the patient’s role in managing their condition. Our occupational therapists are experts in helping patients to regain or utilize adaptive techniques to re-master skills needed for daily living and gainful employment. Therapy provided includes bowel/bladder management, bathing and personal hygiene needs; identifying and learning to use medical equipment and assistive devices; and community mobility and job performance.

Skilled Nursing

Skilled nursing care is a high level of medical care that must be provided by trained individuals such as registered nurses (RNs) and Licensed Practical Nurses (LPNs). Premier Point offers skilled nursing care service that can include:

  • Catheter or Ostomy Care
  • Wound Care
  • Geriatric Case Management
  • Administering medications, in-home therapy, and other pain management treatment
  • Coordinating physical and occupational therapy if needed


Therapy

Many patients have complex health care need and are struggling with decreased functionality due to stroke, neurological diseases, amputation and other disabilities, or aging. Dependent on the individual patient need, family or caregivers oftentimes want to know whether the person can function independently and safely. Premier Point's on-staff physical, occupational, and speech therapists will work with your doctor to create a personalized therapy plan that meets your specific needs.



Dieticians

Our registered dietitians use their nutrition expertise to counsel patients undergoing treatment for a variety of medical conditions including diabetes, heart disease, high cholesterol, high blood pressure, and gastrointestinal disorders on how to make unique, positive lifestyle changes that support their medical care plans.



Medical Social Work

Medical Social Workers help patients and their families with a variety of non-medical needs affecting their well-being. Medical Social Workers are mental health professionals who can provide counseling and referrals for patients if needed.
